Payment:
MasterCard, VISA, Discover, American Express, PayPal, Company Check, or prepaid by Money Order. Open account available for qualified institutions and companies. US funds only.

International Orders:
Overseas customers not using a charge card may prepay their order with international bank check or money order payable through U.S. bank in U.S. funds. Organ Service Corporation reserves the right to request pre-payment by above options only.

Minimum Order:
US$15 minimum for merchandise, shipping extra.

Prices/Quantities:
Prices subject to change without notice. Quantities may be limited. We are not responsible for typographical errors.

Delivery:
Orders are shipped FOB origin. Normal method is via Post Office or United Parcel Service (UPS) ground for the lower 48 states. UPS requires street addresses (no PO boxes, APOs, FPOs, MPOs). Specify instructions for international orders. Hawaii and Alaska via Post Office or UPS blue. Please specify for UPS next day, 2nd day or 3rd day.

Shipping Charges:
UPS chart plus handling and insurance charges will be billed at cost.

Back Orders:
We stock most items listed in this catalog. Any item not available for immediate shipment will be back ordered. Please advise us if you do not wish to accept or pay for a partial shipment.

Claims and Returns:

No collect or COD returns.
All returns must be made within 30 days of the invoice date.
All returns must be in the original packaging and in resalable condition.
Unopened items returned due to customer error will be subjected to a 15% restocking charge.
Circuit boards removed from their sealed anti-static bags may only be returned for 50% credit at the discretion of Organ Service Corporation.
Service manuals, owner's manuals, special order items, and custom packages are non-returnable.
Defective merchandise will be replaced if it is returned within 30 days of receipt and accompanied with a copy of the invoice. This warranty does not apply to items which have been subjected to misuse, abuse, neglect, accident or modification or which have been soldered or altered during assembly. In no event shall our liability exceed the buyer's purchase price.
No refunds on shipping charges on returns.

Sales Tax:
Please be advised that we collect and remit sales taxes only to the state of Indiana. If your purchases are subject to sales tax in another state, it is your responsibility to remit the proper tax to that state.
